By signing this form, you acknowledge and agree to participate voluntarily in coaching or mentoring services delivered by MedComm Consulting Private Limited (“the Coach”).

1.⁠ ⁠Nature of Services

I understand that coaching/mentoring:

• is not medical advice, diagnosis, treatment, or therapy

• does not create a doctor–patient or clinical supervisory relationship

• is intended for educational and professional development only

2.⁠ ⁠My Responsibilities

I take full responsibility for:

• all academic, career, and professional decisions

• attending sessions punctually

• applying advice at my own discretion

3.⁠ ⁠Confidentiality

Information shared will be kept confidential except where:

• required by law

• there is risk of harm

• I give written consent

4.⁠ ⁠Data Protection

I understand my data will be processed according to the Privacy Policy.

5.⁠ ⁠Fees & Cancellation

I agree to the Booking & Cancellation Policy provided to me.

6.⁠ ⁠Liability

I acknowledge that outcomes cannot be guaranteed and I am responsible for all decisions taken.

BOOKING & CANCELLATION POLICY

1.⁠ ⁠Booking

• Sessions must be booked in advance via the website or by direct communication.

• Payment is required to confirm the booking unless otherwise agreed.

2.⁠ ⁠Cancellations by Client

• Cancellations with less than 48 hours’ notice may be non-refundable.

• No-shows are non-refundable.

• Programme fees may be non-refundable once started.

3.⁠ ⁠Rescheduling

You may request to reschedule a session with at least 48 hours’ notice.

4.⁠ ⁠Cancellations by MedComm Consulting

If we must cancel or reschedule, you will be offered:

• an alternative date, or

• a full refund

5.⁠ ⁠Late Arrival

Sessions cannot be extended beyond the scheduled duration.

Arriving late may reduce your session time.

6.⁠ ⁠Group Programmes & Courses

Group programmes follow fixed schedules; missed sessions cannot be refunded or repeated.
